These documents are available to Optometrists only and are designed to be used when offering Optometric Vision Therapy through telehealth. If you would like access to these documents please complete the registration form. Once your registration has been approved you will be sent information on how to log in to the "members section" of the website. There you will be able to complete your purchase.
